Triiodothyronine, known as T3, is an important thyroid hormone that regulates metabolism and ensures your body can perform essential functions properly. While most T3 in the bloodstream is bound to protein, some is unbound or free.
The total T3 test measures the bound T3 and free T3 combined.
Total T3 may be ordered to assess the thyroid function or if a thyroid disorder is suspected. It is also used to assess the pituitary gland problems, evaluate the severity & the type of thyroid disease, and to monitor treatment for a thyroid condition.

A Total T3 test may be ordered in several conditions to assess the Total T3 levels:
- Evaluation of Thyroid Function: If a patient presents with symptoms of thyroid dysfunction, such as fatigue, weight changes, or changes in mood, a T3 Total test may be ordered to evaluate thyroid function and assess for imbalances.
- Assessment of Thyroid Hormone Production: T3 is produced by the conversion of thyroxine (T4) in the body. In cases where there is suspected alteration in thyroid hormone production, such as hyperthyroidism or hypothyroidism, a T3 Total test helps assess T3 levels and contributes to the overall evaluation of thyroid function.
- Monitoring Thyroid Treatment: For individuals receiving thyroid hormone replacement therapy, a T3 Total test helps monitor the effectiveness of the treatment and ensures appropriate thyroid hormone levels.
Your doctor may also recommend this test if you have an abnormal TSH (Thyroid-stimulating hormone) or you are experiencing the symptoms of an overactive thyroid, called hyperthyroidism, which may include:
- Weight changes, usually weight loss
- Fatigue
- Frequent bowel movements
- Hair loss
- Increased appetite
- Increased sweating
- Menstrual irregularities
- Heart palpitations
- Insomnia
- Nervousness or anxiety
- Problems with concentration
Sometimes, it is less helpful in diagnosing hypothyroidism because T3 is typically the last hormone to become abnormal if you are hypothyroid.
